Rare Case of Orbital Cellulitis in a 6-Year-Old Girl Traced Back to Dental Infection
A 6-year-old girl from the West Bank recently experienced a rare case of orbital cellulitis, a serious eye infection that can lead to vision loss and life-threatening complications. The girl's ailment was traced back to a dental infection, a scenario that is uncommon in children.
The girl initially complained of a toothache near her back-right molars. Over the next few days, her condition worsened. Her right eye began to bulge from the socket, she developed double vision and light sensitivity, and the area around it was severely swollen, shiny, flushed, and warm to the touch. She also experienced a headache, swelling around her right eye, fever of 104 degrees Fahrenheit (40 degrees Celsius), and two episodes of vomiting.
A CT scan of the girl's head showed that her right sinus cavities were so inflamed they appeared opaque on the scan. Ophthalmologists determined that the girl's eye swelling was caused by a bacterial infection known as orbital cellulitis. Bacteria had migrated from the girl's original infection in her teeth to her sinuses, and had then spread to her eye.
In children, orbital cellulitis is usually caused by sinus infections, not dental issues. However, rare cases like this one have been documented in medical literature, albeit very few. To date, only about four documented cases link dental origins to orbital cellulitis in children[1][2].
The typical mechanism involves bacteria from an infected tooth migrating first to the sinuses, then spreading to the orbital tissues around the eye. This leads to an aggressive infection of the soft tissues surrounding the eye.
Since the development of a vaccine for H. influenza type b, orbital cellulitis is usually caused by S. aureus (also known as staph) and various species of Streptococcus. Before 1985, orbital cellulitis was commonly linked to the bacterium Haemophilus influenza.
The girl was admitted to a hospital pediatric ward and received intravenous antibiotics, including vancomycin, for her infection. Treatment often includes surgical drainage of abscesses to relieve eye pressure, extraction of infected teeth contributing to the spread, sinus drainage or removal of polyps if involved, and intravenous broad-spectrum antibiotics targeting likely bacterial agents.
The rarity of dental origin orbital cellulitis in children means clinicians primarily consider sinus infections but should remain aware of dental infections as a potential source in persistent or atypical cases[1][2][5]. This case serves as a reminder of the importance of prompt and appropriate medical intervention in such cases.
